As part of your childs Portland Public Schools educational program, (s)he may have the opportunity to publish documents and projects on our district and school pages on the World Wide Web, and possibly in other public forums (examples might include: PowerPoint presentations, posters, Portland Partnership website, TV3 website, teacher Bboards, etc.). These documents and projects will include class assigned work, pictures, and /or videos associated with school-based activities. We will publish such work only after receiving your written permission.

 

Please review the following information, indicate your choices, sign the form and return it to your childs teacher as soon as possible. Thank you.

 

Some safeguards we observe are:

        Each page within a school's website must identify the name of the school.

        Each page within a school's website will have a link back to its home page.

        Parent or guardian permission to use a student's picture must be obtained and kept on file by the Web Publishing Coordinator.

        Parent or guardian permission to use unique student-generated content or products must be obtained and kept on file by the Web Publishing Coordinator.

        Students may only be identified by first name and initial of last name.
